Application Process

All applications must be turned in by Sunday, January 31, 2010, to a Student Ministries staff member. Incomplete applications will not be accepted. Late applications will be placed on a waiting list if space allows.
 

All students must fill out the following items to apply for a mission journey:
Application
• Reference form (1) (may come separate)
• Parent Reference Form (1)
• Mission Journey Agreement Form
• Marking of journey preferences
• A notarized Bent Tree Release Form (if not on file)
• Deposit (amount specified by journey)
• Copy of student photo ID (i.e., passport, driver’s license, school ID) Please copy at 110%
• Copy of medical insurance card (both sides)

 
 

Directions for the application process:
1. Pray about God’s plan for the spring and summer and whether or not a mission journey might be a correct fit.
2. Read the journey information pages to determine which journeys you are eligible to apply for and check the tentative Mission Training Meeting schedule on btbf.org/studentmissions to make sure you can attend all the meetings.
3. Find your grade level at time of application and place a 1 next to your top choice. Place an x next to all other journeys you are available to go on. List a reason for any journey that you do not mark. Please be aware that only a small percentage of students will be able to receive their first choice.
4. Both parents with legal guardianship must sign the mission trip agreement form regardless of current mental status. We do accept fax signatures. Our fax number is 972-428-1383.
5. One parent must complete the Parent Reference form and the Agreement form and ensure that the notarized medical release form is on file. A Notary Public is available at the main church office during the week.
6. Student should complete his/her section of the application and read
and agree to the Mission Journey Agreement form.
7. Determine the deposit amount required for student’s first choice journey and submit this with the application. Funds will be transferred to a different journey if necessary. Please submit separate checks for each student applying. Checks will not be processed until after the parents’ meeting on February 21.
